• Recent Fall
• Past History of Falls
• Risk of Falls
• Fear of Falls

2. Must live in one of the following areas:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

$7 per session to attend.
