1. Must have one (or more) of the following health problems:
- Recent Fall
- Past History of Falls
- Risk of Falls
2. Must live in one of the following areas:
- City of Brimbank
- City of Hobsons Bay
- City of Maribyrnong
- City of Melton
- City of Wyndham
3. Must have a referral from one of the following:
- Hospital or community-based health professional
- General Practitioner (GP)
Cost
There is no cost to attend the Falls and Mobility Clinic.
