Eligibility

1. Must have one (or more) of the following health problems:

• Recent fall
• History of falls
• Risk of falls
• Fear of falls
• Severe dizziness resulting in falls or risk of falls

2. Must live in one of the following areas:

• Moreland
• Hume
• Whittlesea
• Banyule
• Nillumbik
• Darebin

3. Must have a referral from one of the following:

• Hospital or community-based health professional
• General Practitioner (GP)
• Self-referral or referral from family members are accepted

Cost

The cost to attend the Falls and Balance Service Program is between $5 – $10 based on individual circumstances.
