• Accredited and Registered Fitness Instructor (Certificate 3 & 4 and a Diploma in Fitness instruction)

ELIGIBILITY

1.      Must have one (or more) of the following:

• Recent fall
• Past history of falls
• Risk of falls
• Fear of falls
• 65 and older age criteria

2.      Must live in one of the following areas:

• Hospital-based or Community-based health professional
• Consultant
• Registrar/Resident
• Nurse
• Allied Health Professional
• Care Co-ordinator
• Aged Care Assessment Service
• Case Manager
• General Practitioner (GP)
• Self-referral or referral from family members are accepted
